New Patient Forms and Health History
Center for Progressive Dentistry is a completely paperless office. All new patient forms and health history questionnaires will be emailed to you prior to your appointment and can be submitted online. If the forms are not received ahead of time, please arrive at least 15 minutes before your first appointment in order to complete these forms.
